CBH PROVIDER DIRECTORY
The Provider Directory is a comprehensive listing of all providers in the CBH network. The Directory, as it appears on this website, is divided into three parts. Each part, in turn, is comprised of several sections. The first part consists of topical indexes summarizing each provider. The second division explains the abbreviations used in the Directory. The third major section gives a detailed view of each provider, listed alphabetically by organization name.
In both the summary and detail views, the information is self-reported to CBH by each provider. The hours of operation, languages spoken, and special services with which a provider has experience may have changed since that information was last reported.
This website version of the Provider Directory will be updated on a regular basis. We encourage you to offer any suggestions you may have to help us improve our site. You can do so by clicking on the Contact Us link on the left side of this page.
