
YOUR RIGHTS
Confidentiality | Your Responsibilities
As a member of CBH, you have certain rights:
- You have the right to be treated with dignity and respect.
- You have the right to confidentiality.
- You have the right to look at your records.
- You have the right to have your treatment plan
explained to you.
- You have the right to help set up your treatment plan.
- You have the right to refuse to follow your treatment
plan, knowing the risks you might be taking.
- You have the right to complain if you are unhappy with
the services you are using.
- You have the right to change your counselor.
- You have the right to know the qualifications and job
description of any person who is helping you.
- You have the right to "Notice and Appeal" which means
that if a service is denied you must receive a letter that
tells you the service is denied. AND, you have the right
to appeal (ask for another review of) that decision (Complaints and Grievances).
