Schedule As are now available through the EDI Browser 

Since using incorrect billing codes is one of the most frequent claims submission errors, we strongly encourage you to refer to your most recent Schedule A.  Copies of the most current version will always be available through the EDI Browser interface.

The Schedule As from the EDI Browser are in Adobe PDF format.  To view and print these files, you will need the free Adobe Acrobat Reader software. For more information, see our help page

We are successfully adjudicating.

Please submit all claims as you normally would, particularly claims you may have held back.

EDI Confirmation Reports are now available through the

EDI Browser interface in the Download / Reports section.

CBH is still addressing some internal system performance issues, but we are adjudicating.
Current Top Two Claims Submission HIPAA-related Errors
  • Incorrect Provider Number:  Providers should submit claims with the 13 digit PROMISe provider number listed on the Schedule As mailed last week.
  • Incorrect Billing Codes:  Providers should submit claims using the billing codes contained in the Schedule A forms mailed last week.  All BANs (modifiers attached to unauthorized services) are unique.

So that we can process claims more efficiently, we recommend that providers limit files to 2 MB in size.  You may still upload multiple files within a .zip file but individual files within the .zip file should not exceed the 2 MB recommendation.

Please make sure you carefully read the letter which accompanied your most recent payment from CBH.

We are currently testing the modifications we have made to our system to fix the internal logic errors we found.  If successful, we will begin adjudication as soon as possible.
April 2, 2004
CBH is still working to resolve the internal system logic error.  Electronic files received between March 13 and April 2 have been processed and will go through the next adjudication.
Paper claims have been entered into the system and will be adjudicated with the electronic files
All 997, TA1, and submissions reports will be available for provider review on the day before the next adjudication.
CBH will fax providers EDI Confirmations letters today. This new report, based on the old EDI confirmation letter, was developed in response to provider requests.
We will release new CBH Companion Guides to the 837P and 837I on or about April 15. If future revisions of the Companion Guides are published, they will be available on the 1st or 15th of the month.
