Most of the following forms are available in three formats.
"MS Word Document" format is designed for Microsoft Word users. Clicking on this link allows users to fill out forms using their personal computers and save them as .DOC files. Forms can then either be kept on your hard drive or e-mailed to CBH as attachments.
The "RTF Document" format is designed for non-Microsoft Word users. This version also allows users to fill out forms using their personal computers, and save them as .RTF files, which will open in a variety of other programs, as well as Microsoft Word. This format as well allows users to save forms on their hard drive or attach them to e-mail.
The "Adobe PDF Document" format is an Adobe Acrobat Reader format. This version can only be viewed online and is not designed to be filled out electronically. Forms can be printed from a personal computer, filled out manually, and mailed to CBH.
